Option 1: Print Application
• Download the application.
• You can then print it and fill it out by hand or you can fill it out using Acrobat Reader and then print it.
• Mail it along with $50 and a recent photo of the applicant to:

Click Submit at the bottom. This will send your information to us directly through our website |

You will then be taken to a page where you can submit a digital photo of the applicant and pay your $50 application fee. (Please note, we do not accept debit cards at this time.) |

Please note that both options require Adobe Reader. We highly recommend you print a version of the application before submitting it.
